Whānau Ora Navigator - Te Kotahi o Te Tauihu Charitable Trust Nelson/Blenheim
Fixed term (1 July 2025 to 31 December 2025) - One role for Nelson, one role for Blenheim, 40 hours per week.
Te Kotahi o Te Tauihu Charitable Trust is mandated by the eight Iwi of Te Tauihu o Te Waka- a-Māui. Our overarching purpose is to create the best conditions for our whānau to thrive in alignment with Kotahi te Hoe Strategy.
The Whānau Ora Navigator will provide culturally based, whānau-centered support to whānau in the Nelson/Tasman and Marlborough regions, helping them transition from crisis or distress towards wellbeing. The Navigator will work collaboratively with whānau to identify their goals, connect them with necessary services, and support them in achieving better outcomes in areas such as mental health, education, housing, employment, and cultural identity.
JOB DESCRIPTION
Whānau Support:
Engage with whānau to assess their needs and aspirations.
Develop and implement tailored whānau plans to address barriers and achieve goals.
Provide ongoing support, advocacy, and guidance to whānau throughout their journey.
Service Coordination:
Connect whānau with appropriate services and resources (e.g., health, education, housing, employment).
Collaborate with local organisations, iwi, hapū, and community groups to ensure wraparound support.
Monitor and track whānau progress, adjusting plans as needed.
Cultural Leadership:
Incorporate Te Ao Māori principles and practices into service delivery.
Foster cultural identity and connection for whānau, including links to whakapapa, marae, and community.
Data and Reporting:
Collect and record whānau information, including IDI Data and Whānau Ora Progression Data, in accordance with the Agreement.
Submit timely and accurate reports to the Commissioning Agency as required.
Compliance and Best Practice:
Adhere to all policies, procedures, and legislative requirements, including the Children’s Act 2014, Privacy Act 2020, and Health and Safety at Work Act 2015.
Maintain confidentiality and handle sensitive information with care.
Continuous Improvement:
Participate in training and professional development to enhance skills and knowledge.
Contribute to the evaluation and improvement of Whānau Ora Services.
Key Relationships
Internal: Service Provider Manager, other Navigators, and support staff.
External: Whānau, Commissioning Agency (Te Tauraki Limited), Te Puni Kōkiri, local service providers, and community organisations.
Person Specification
Essential:
Experience working with whānau or communities in a support, advocacy, or social services role.
Strong understanding of Te Ao Māori and tikanga, with the ability to integrate these into practice.
Excellent communication, interpersonal, and problem-solving skills.
Ability to build trusting relationships with whānau and work collaboratively with stakeholders.
Proficiency in data collection and reporting.
Commitment to the principles of Whānau Ora and improving whānau wellbeing.
Full clean driver’s licence.
Competent in Microsoft products e.g. WORD, EXCEL, OUTLOOK.
Excellent CV and Cover Letter writing.
Excel in accurate, timely, daily reporting.
Desirable:
Qualification in social work, community development, or a related field.
Experience working with government agencies or in a contracted service environment.
Knowledge of local services and networks in Nelson.
Conditions of Employment
Must complete criminal conviction history checks and safety checks under the Children’s Act 2014.
Must adhere to the Service Provider’s policies, including health and safety, privacy, and conflict of interest.
Must comply with the terms of the Whānau Ora Services Agreement, including reporting and performance requirements.
